So Workable Solutions can give you the best service possible, there are a few things we'll ask you to do.
• Please arrive 15 minutes before the appointment to complete any paperwork or insurance forms relating to your injury.
• Wear comfortable clothing and shoes (such as athletic shoes).
• Please bring a pair of shorts if you have a knee or lower extremity injury.
• Take all medications as you would normally take them prior to the evaluation, unless it impairs your ability to drive.
• If you will be involved in any evaluation or treatment of a work related injury, please click on the Client Intake Questionnaire link below and complete it as best as you can. Your therapist will review it with you at the time of your evaluation.
